If there are any changes to your past medical history or your contact info; address, telephone, or a new health card, please update us. It is your responsibility to provide us with up to date information.

In accordance with CPSO (College of Physicians and Surgeons of Ontario) & OMA (Ontario Medical Association) Guidance, The "No-show” fee is $40 .


Should you need to cancel or reschedule an appointment please contact our office as soon as possible; no later than 24 hours prior to your scheduled appointment.

Make sure to present your valid Health Card to the front desk, if you do not have one you will be charged for your visit.

Please wear a mask and arrive on time.

Bring any medications you are currently taking in the original packaging. It is useful for the doctor in case there is a need to change the dosage or stop the medication.

It is always recommended to wear loose sleeves for blood pressure checks and injections.

If your condition seems to not be improving, book a follow-up appointment either by office visit or by a phone call.

It is recommended that you contact our office 2 weeks after you have done your imaging or blood tests to check for results in case you have not heard from us. Sometimes we don't receive the results directly and patients assume that no news is good news.
